Applications for MPSI ECC may be obtained from the MPSI ECC office. Applications will be accepted and processed as they are received. In addition to the application, the following will be required before the student can begin attending MPSI:
o Copy of your child’s birth certificate
o Application with $50.00 non-refundable registration fee.
o Health appraisal form (due within 30 days of initial attendance. It must include current information on immunizations and be signed and dated by your child’s physician/clinic)
o Child’s immunization information (or immunization waiver form)
o Child Information Record
o Copy of child’s health insurance card
o Tuition deposit (first week’s tuition, if applicable)
o Photo/Video/Digital Release form
o Other documentation as required by specific tuition assistance programs
